Bridging the Postpartum Gap in California

More than half of maternal deaths in California occur between one week and one year postpartum, underscoring the need for stronger transitions from maternity care to ongoing primary care.

To help close this gap, the California Quality Collaborative convened the California Primary Care Postpartum Clinical Workgroup to develop practical guidance for improving postpartum-to-primary care transitions. Together, these resources clarify clinical ownership, transition timing, essential handoff information and condition-specific follow-up for gestational diabetes, type 2 diabetes, hypertensive disorders of pregnancy and maternal mental health.
