Yet ensuring patients receive recommended vaccines requires more than making vaccines available. Primary care practices need reliable data, effective workflows, proactive outreach and clear communication to identify vaccination gaps and connect patients with needed immunizations.
CQC works with primary care practices and healthcare partners to strengthen immunization delivery through practical, practice-level improvement. By helping care teams identify operational barriers, strengthen workflows and use data to guide improvement, CQC is supporting more consistent and proactive delivery of recommended vaccines across California.
