Our final convening brought together participants and partners to share reflections on their transformation journeys, celebrate the many connections formed and recognize the achievement of our North Star.

The Strength of Our Connections

Grounding

 

Connections built during the EQuIP-LA will outlast the program. In recognition, our first session of the day was dedicated to celebrating the many bonds and friendships developed over the last two years.

Craft Activity

 

Participants were given a blank puzzle piece to commemorate a connection they formed over the last two years that will remain with them after the program ends.

Sharing Our Connections

 

After creating their puzzle piece, each participant shared the connection they chose and why, and how this connection will continue to influence their future work. The different types of connections included:

• A realization about themselves
• A relationship that impacted them
• A moment when they linked ideas or learned something new
• A professional connection that supports their work

Piecing Together Our Connections

 

Participants reflected on our puzzle of collective connections and noticed how the different types of connections (personal, intellectual, professional) support each other.  Just like a puzzle, each connection—whether a personal insight, a relationship, a learning moment, or a professional bridge—come together to complete the bigger picture of our program’s growth and impact.

Provider organizations used the art of storytelling to share their team's EQuIP-LA journey and path of transformation.

Practice Stories of Transformation

Not only did we hear each PO team’s story of transformation, but we also heard from the practices themselves.

Reddy Care Medical’s video highlighted how EQuIP-LA greatly enhanced their care coordination and patient follow-up.

 

Tweedy Medical Group’s video credited EQuIP-LA with invigorating their efforts to better serve their patients and greatly expanding their goals.

Fireside Chat

Melissa Preciado facilitated our final session of the day, a fireside conversation with EQuIP-LA’s partners, where panelists reflected on the past, present, and future of EQuIP-LA.
