Who Should Submit

We welcome proposals from individuals and organizations across the healthcare ecosystem, including those working at the intersection of care delivery, policy, community engagement and innovation.

Proposal Requirements

  1. Address a Core Focus Area: Align your session with at least one focus area (listed below), keeping innovation front and center.
  2. Highlight Collaboration: Demonstrate how purchasers, payers, providers, community organizations and/or patients work together to drive success.
  3. Feature Multiple Perspectives: Sessions must include 2 speakers, up to a maximum of 4, from multiple organizations who bring diverse stakeholder perspectives to the discussion.

2027 Focus Areas

  1. Accelerating sustainable investment in primary care and behavioral health: Innovative financing, policy, and delivery approaches that increase and sustain investment in primary care and behavioral health, strengthen care teams, and improve access and outcomes.
  2. Advancing value-based care through alignment and accountability: Strategies that move value-based care from aspiration to practice through multi-payer alignment, accountable care models, aligned measurement, alternative payment approaches, or redesigned systems of care.
  3. Building a connected, technology-enabled healthcare ecosystem: Partnerships, infrastructure, data-sharing strategies, artificial intelligence, automation, digital health, and other emerging technologies that reduce administrative burden, improve coordination and care delivery, strengthen workforce capacity, and make information more actionable for patients, providers, payers, and communities.
  4. Partnering with communities to advance health equity: Approaches that meaningfully engage patients, families, and communities in the design and delivery of care, address social and structural barriers, strengthen cross-sector partnerships, and improve equity.
  5. Promoting appropriate, equitable, and person-centered use of care: Strategies that reduce the overuse, underuse, and misuse of healthcare services while helping patients receive the right care, at the right time, in the right setting.
  6. Strengthening maternal, child, and adolescent health: Strategies that strengthen maternal, preventive, and behavioral healthcare across pregnancy, childhood, and adolescence, including immunizations, developmental screening, well-child visits, and adolescent health and well-being.

Session Formats

We invite a range of session formats, including panels, case presentations, interactive workshops and creative approaches that foster dialogue, peer learning and practical takeaways.

Sessions should be designed for a 60-minute time slot (e.g., ~30 minutes of presentation + ~30 minutes of discussion).

Review Process

Proposals will be reviewed and scored by a committee of CQC team members and Steering Committee representatives. Selected sessions will be chosen based on relevance to the theme, alignment with focus areas, clarity of objectives and potential for audience engagement.
