Strengthening the Primary Care Workforce: Embedding Community Health Workers Within Care Teams
Community health workers (CHWs) are a powerful bridge between the clinical care team and the communities they serve, helping to address social drivers of health and improve outcomes for underserved populations. CQC has long recognized the value of CHWs as part of team-based care and continues to support innovative strategies for embedding them into primary care settings.
This implementation spotlight highlights the integration of CHWs at Reddy Care Medical, a small, independent practice participating in CQC’s EQuIP-LA program. It offers a practical example of how primary care teams can train and implement CHWs to improve patient engagement, care coordination and health equity.
