Care Management Plus Training for Care Managers

Overview

CQC is partnering with Oregon Health and Science University (OHSU) to offer their Care Management Plus Training program in California to support improved care management for patients with chronic diseases. The Care Management Plus Curriculum includes an in-person intensive training and subsequent 8 weeks of online instruction. Trainings will provide participants with the knowledge and tools necessary to better manage a chronically ill patient population.

The in-person component provides an intensive jump-start to the core competencies of care management. The rest of the Care Management Plus Curriculum is taught online over 8 weeks through learning modules, discussions, and assignments, and each week's material can be reviewed at the convenience of the student after completion.

The Care Management Plus Training is August 31 - September 1 in the Los Angeles area. The registration deadline is July 22, 2011. We suggest you register as early as possible as space is limited.

  • The training fee is $1,500 per individual, which includes the in-person training and the 8 weeks of follow-up online instruction.

Who should attend

The training is designed for those directly involved in care management - usually a Care Manager/Care Coordinator, Lead Provider, Clinic Manager, and/or others. Although the Care Management Plus program includes specialized information technology tools, in particular ICCIS (Integrated Care Coordination Information System), use of that technology is optional and the in-person training is focused on defining the roles and responsibilities of a care manager and providing them with the skills necessary to better manage a chronically ill patient population (see the link to the draft agenda below).

Care Managers who successfully complete the Care Management Plus Curriculum will gain skills to:

  • Teach patients with multiple chronic diseases to organize, prioritize, and implement suggested self-management strategies
  • Identify barriers to care and intervene to overcome or eliminate these when possible
  • Coordinate resources to ensure that necessary services are provided at the most appropriate level of care and at the appropriate time
  • Identify patient situations at-risk for destabilization and intervene to eliminate the risk when possible

Healthcare Education Services at OHSU is an accredited approver by the American Nurses Credentialing Center's Commission on Accreditation. Oregon Health & Science University School of Medicine is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.
