Building Care Solutions for Older Adults with Complex Needs (BCS)

A new opportunity to learn from peers, access training and receive customized technical assistance from national experts begins in January 2017.

Learning from our experience implementing the IOCP, we know that developing programs for the medically complex poses sizable challenges:

  • Targeting the right people
  • Engaging patients and practices in programs
  • Integrating social determinants of health and behavioral health
  • Building a program that has a ROI
  • Connecting to other care management programs such as palliative care and case management

With funding from The SCAN Foundation, CQC is launching a new program, providing participants with the following benefits:


The ideal program participant is an organization that:

  1. Serves a high proportion of Medicare beneficiaries or dual-eligible beneficiaries
  2. Desires to integrate a more person-centered approach to patient care
  3. Wants to share and learn with other organizations
