Join the Program

Participation Requirements

  • Assemble a team of a minimum of 2 including a physician leader (CMO, Medical Direcor, or Clinical Champion) and project manager
  • Attend four onsite learning sessions, 1-2 days long and monthly teleconferences
  • Submit data quarterly on selected measures and populations from registry via the Collaborative Reporting Template
  • Share openly with other participating groups about what you are measuring and how you are trying to improve patient care
  • Provide evidence of CEO support

Cost

Program cost is based on total HMO enrollment. Each team is responsible for travel and lodging expenses.

  • Program fee - $3,500 per organization under 60,000 enrollees and $7,000 for organizations over 60,000 enrollees
  • Gain sharing - refundable program fee for making at least 20% improvement from a baseline measure toward 100% on at least one relevant measure during the program
