Reporting Timeline

• M1: DOS Jan 1 through June 30, 2008
  • Submit to Collaborative by Sept 15 (note: 60 day claims lag)
• Progress Report #1
  • Submit to Collaborative by Sept 15 (note: template available by August 8)
• M2: DOS July 1 through Sept 30, 2008
  • Submit to Collaborative by January 15
• Progress Report #2
  • Submit to Collaborative by January 15, 2009
• M3: DOS October 1 through December 31, 2008
  • Submit to Collaborative by April 15
• M4: DOS January 1, 2009 through March 31, 2009
  • Submit to Collaborative by July 15

** Due to the time lag in reporting outcomes and the need to assess progress earlier in the process, the progress reports will ask you to report on your experiences with testing interventions and meetings with physicians. Experiences reported in the progress reports will help you and the Collaborative gauge how your team is progressing in the measures and interventions you selected.
